Avacincaptad pegol + ranibizumab is a combination therapy investigated for the treatment of neovascular age-related macular degeneration (nAMD). Avacincaptad pegol is a pegylated RNA aptamer that specifically binds to complement component C5, inhibiting its cleavage into C5a and C5b, thereby blocking the terminal steps of the complement cascade. This action reduces membrane attack complex (MAC) formation and subsequent retinal pigment epithelium (RPE) disruption, aiming to slow photoreceptor and RPE atrophy. Ranibizumab is a monoclonal antibody fragment that inhibits vascular endothelial growth factor A (VEGF-A), reducing abnormal blood vessel growth and leakage in the retina. The combination targets both complement-mediated inflammation/atrophy and VEGF-driven neovascularization in nAMD[1][2][4].
