Overview
Avacopan is an orally administered small-molecule drug used as an adjunctive treatment for adults with severe active anti-neutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, specifically granulomatosis with polyangiitis and microscopic polyangiitis. It acts as a highly selective antagonist of the complement 5a receptor (C5aR, also known as CD88), blocking C5a-mediated neutrophil activation, migration, chemoattraction, and adhesion. This targeted mechanism reduces blood vessel inflammation while preserving other beneficial complement pathways such as the membrane attack complex (MAC). Avacopan is not approved for primary therapy but is used in combination with standard immunosuppressants and corticosteroids to improve disease outcomes.
