A combination of **avacopan** and **tofacitinib**, two orally administered small molecule drugs. Avacopan is a selective antagonist of the complement 5a receptor 1 (C5aR1), blocking the binding of C5a and thereby reducing neutrophil activation and inflammatory processes associated with anti-neutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis[1][8][14]. Tofacitinib is a Janus kinase inhibitor, primarily targeting JAK1 and JAK3, which blocks the JAK-STAT signaling pathway and inhibits cytokine signaling central to autoimmune and inflammatory diseases like rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis[2][5][6][12]. The combination has theoretical immunomodulatory effects from inhibition of both complement-mediated neutrophil activation and cytokine-driven immune cell activation.
