Overview

Avelumab + cabozantinib is an investigational combination therapy for cancer, primarily being studied in metastatic renal cell carcinoma (mRCC) and metastatic urothelial carcinoma. Avelumab is a fully human IgG1 monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and B7.1 receptors, thereby enhancing T-cell mediated immune responses against tumor cells. It also mediates antitumor activity via natural killer (NK) cell-mediated antibody-dependent cell-mediated cytotoxicity (ADCC). Cabozantinib is an oral small molecule tyrosine kinase inhibitor targeting multiple kinases including MET, VEGFR, and TAM family receptors involved in tumor growth, angiogenesis, and immune regulation. The rationale for combining these agents is to harness their synergistic effects—avelumab’s immunomodulatory action with cabozantinib’s antiangiogenic and immunomodulatory properties—to improve disease control rates and survival outcomes in patients with advanced cancers[1][3][5].
