FLOT-A is a peri-operative chemo-immunotherapy regimen investigated for the treatment of resectable gastro-oesophageal adenocarcinoma (GOA). The regimen consists of the standard-of-care FLOT cytotoxic chemotherapy backbone—comprising **fluorouracil**, **leucovorin** (folinic acid), **oxaliplatin**, and **docetaxel**—combined with **avelumab** (Bavencio), a human anti-PD-L1 monoclonal antibody. Avelumab functions as an immune checkpoint inhibitor by blocking the interaction between PD-L1 and its receptors (PD-1 and B7.1), thereby restoring T-cell-mediated anti-tumor immune responses. The rationale for this combination is to leverage the pro-immunogenic effects of FLOT chemotherapy, which can induce immunogenic cell death and increase tumor antigen presentation, to enhance the efficacy of PD-L1 blockade. The regimen was primarily evaluated in the Phase II ICONIC trial (NCT03399071), led by the Royal Marsden NHS Foundation Trust in collaboration with Merck KGaA.
