Avelumab + gemcitabine + cisplatin is a combination therapy used primarily in the treatment of advanced or metastatic urothelial carcinoma (bladder cancer) and muscle-invasive urothelial carcinoma. Avelumab is a monoclonal antibody that targets programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and B7.1 receptors on T cells, thereby enhancing anti-tumor immune responses. Gemcitabine and cisplatin are cytotoxic chemotherapeutic agents; gemcitabine is an antimetabolite that inhibits DNA synthesis, while cisplatin forms DNA crosslinks leading to apoptosis. This combination has shown promising efficacy in clinical trials as both neoadjuvant and first-line therapy for bladder cancer, improving event-free survival and overall survival rates compared to chemotherapy alone[1][2][3][6].
