Overview

Avelumab + ipilimumab + pembrolizumab is an investigational combination of three immune checkpoint inhibitor monoclonal antibodies. Each component targets a different immune regulatory pathway to enhance anti-tumor immunity: - **Avelumab** is a fully human IgG1 monoclonal antibody that binds to programmed death-ligand 1 (PD-L1), blocking its interaction with PD-1 and B7.1 receptors on T cells and antigen-presenting cells, thereby promoting T-cell mediated anti-tumor responses[3]. - **Ipilimumab** is a fully human IgG1 monoclonal antibody that blocks cytotoxic T lymphocyte-associated protein 4 (CTLA-4), enhancing T-cell activation and proliferation[8]. - **Pembrolizumab** is a highly selective humanized IgG4-kappa monoclonal antibody targeting programmed cell death protein 1 (PD-1) on lymphocytes, preventing its interaction with PD-L1/PD-L2 and restoring immune response against tumor cells[7]. This triple combination has not been approved as a regimen for any indication as of June 2025. While dual combinations such as nivolumab + ipilimumab, pembrolizumab + axitinib, or avelumab + axitinib are approved for several cancers including renal cell carcinoma, there are no clinical data or regulatory approvals for the simultaneous use of all three agents together in patients[2][6][9]. The rationale behind combining multiple checkpoint inhibitors would be to synergistically augment anti-tumor immunity by blocking multiple inhibitory pathways; however, this approach may also increase the risk of severe immune-related adverse events.
