Avelumab + pemetrexed + carboplatin is a combination regimen used primarily in the treatment of non-small cell lung cancer (NSCLC) and malignant pleural mesothelioma. Avelumab is an anti-PD-L1 monoclonal antibody that enhances the immune system's ability to detect and destroy cancer cells by blocking the interaction between PD-L1 on tumor cells and PD-1 on T-cells. Pemetrexed is an antifolate chemotherapy agent that inhibits several folate-dependent enzymes involved in nucleotide synthesis, thereby preventing DNA replication in rapidly dividing cells. Carboplatin is a platinum-based chemotherapy drug that causes DNA crosslinking, leading to apoptosis of cancer cells. This combination leverages both immunotherapy (avelumab) and cytotoxic chemotherapy (pemetrexed, carboplatin) for synergistic antitumor effects[3][4][5].
