Avelumab + utomilumab + azacitidine is an experimental combination therapy investigated for the treatment of relapsed/refractory diffuse large B-cell lymphoma (DLBCL). - **Avelumab** is a monoclonal antibody targeting programmed death ligand 1 (PD-L1), acting as an immune checkpoint inhibitor. - **Utomilumab** is a fully human IgG2 monoclonal antibody agonist for 4-1BB (CD137), a co-stimulatory receptor on T cells, intended to enhance antitumor immunity by activating T cells and natural killer (NK) cells. - **Azacitidine** is a nucleoside analog and DNA methyltransferase inhibitor (epigenetic modulator), which can increase cancer cell immunogenicity, potentially sensitizing tumors to immunotherapy. The rationale involves synergistic immune system activation from checkpoint blockade (avelumab), T cell/NK cell co-stimulation (utomilumab), and epigenetic modulation (azacitidine). The regimen was studied in the phase Ib JAVELIN DLBCL clinical trial (NCT02951156, arm B) but showed no clinical responses in this patient population. All agents are administered parenterally: avelumab and utomilumab by intravenous infusion, and azacitidine by subcutaneous injection[1][4][5][6].
