AXER-204 is a recombinant fusion protein biologic designed to promote neural repair and functional recovery after spinal cord injury. It acts as a soluble Nogo receptor-Fc decoy that blocks the binding of three oligodendrocyte-derived inhibitors of axon growth—Nogo-A (RTN4), myelin-associated glycoprotein (MAG), and oligodendrocyte myelin glycoprotein (OMgp)—to the neuronal NgR1 receptor. By preventing these inhibitory proteins from activating NgR1 on neurons, AXER-204 removes barriers to axonal regeneration and plasticity in the central nervous system. The drug is administered intrathecally and has demonstrated safety and preliminary efficacy in both animal models and early-phase clinical trials for chronic cervical spinal cord injury.
