Overview

This investigator-initiated combination regimen, studied in the Phase II EPIC trial (NCT03719989) led by Seoul National University Hospital, evaluates the efficacy of epigenetic priming followed by salvage immunochemotherapy. The treatment consists of azacitidine, a hypomethylating small molecule, administered subcutaneously for five days to enhance chemosensitivity. This is followed by the R-GDP regimen, which includes the anti-CD20 monoclonal antibody rituximab, the antimetabolite gemcitabine, the platinum-based alkylating agent cisplatin, and the corticosteroid dexamethasone. The regimen is specifically designed for patients with relapsed or refractory diffuse large B-cell lymphoma (R/R DLBCL) who are not candidates for autologous stem cell transplantation. The rationale is that hypomethylating agents like azacitidine can sensitize lymphoma cells to subsequent chemotherapy by reversing epigenetic silencing of genes involved in drug sensitivity and apoptosis.
