This combination regimen consists of the hypomethylating agent azacytidine and donor lymphocyte infusion (DLI), specifically investigated for the treatment of relapsed acute myeloid leukemia (AML) and myelodysplastic syndrome (MDS) following allogeneic hematopoietic stem cell transplantation. Azacytidine, a DNA methyltransferase inhibitor, is administered sequentially before DLI to induce epigenetic modifications in leukemic cells, potentially enhancing the expression of tumor-associated antigens and sensitizing the cells to the graft-versus-leukemia (GvL) effect. The subsequent donor lymphocyte infusion provides adoptive immunotherapy by introducing donor-derived T-cells to target and eliminate residual malignant cells. This approach aims to improve clinical outcomes in patients who have failed primary transplant therapy.
