**AZD4573 + acalabrutinib** is an investigational combination therapy currently in Phase 1/2 clinical development for the treatment of relapsed or refractory diffuse large B-cell lymphoma (DLBCL) and other advanced hematological malignancies. AZD4573 is a potent and highly selective small molecule inhibitor of Cyclin-dependent kinase 9 (CDK9), which reduces anti-apoptotic and oncogenic protein expression (such as MCL-1, BFL-1, and MYC), leading to rapid induction of apoptosis in cancer cells. Acalabrutinib is a small molecule inhibitor of Bruton tyrosine kinase (BTK), which increases expression of pro-apoptotic proteins like Bim, supporting apoptosis induction. Preclinical data suggest that the combination accelerates caspase-3 cleavage and induces synergistic apoptosis in malignant cells. Early clinical data show promising response rates and an acceptable safety profile in heavily pretreated patients, including those with prior CAR-T therapy[1][2][3][4][5].
