Overview

AZD5440 is a recombinant, inactive form of human coagulation factor Xa (FXa) currently under development by AstraZeneca. It is designed to function as a reversal agent (antidote) for Factor Xa inhibitors, such as apixaban and rivaroxaban, which are widely used as oral anticoagulants. AZD5440 acts as a decoy receptor that binds and sequesters these inhibitors in the plasma with high affinity, thereby preventing them from inhibiting endogenous Factor Xa and restoring normal thrombin generation and blood clotting. The molecule is specifically engineered to be catalytically inactive (unable to convert prothrombin to thrombin) and lacks the membrane-binding gamma-carboxyglutamic acid (Gla) domain, which ensures that the decoy itself does not participate in the coagulation cascade or promote systemic thrombosis. AZD5440 is currently being evaluated in Phase 1 clinical trials to assess its safety, tolerability, and pharmacokinetics in healthy volunteers.
