The primary mechanism of action is as an inhibitor of pyruvate dehydrogenase kinase (PDK), specifically targeting PDK isoforms involved in the regulation of glucose metabolism. By inhibiting PDK activity, AZD7545 promotes activation of the pyruvate dehydrogenase complex (PDC), thereby enhancing mitochondrial oxidation of pyruvate and shifting cellular metabolism from glycolysis toward oxidative phosphorylation.
