AZD7986 + itraconazole refers to the combination of AZD7986, a small molecule inhibitor of dipeptidyl peptidase 1 (DPP1), and itraconazole, a triazole antifungal agent. AZD7986 blocks DPP1, an enzyme essential for activation of neutrophil serine proteases such as neutrophil elastase, aiming for disease modification in neutrophil-driven inflammatory conditions like COPD and related lung diseases. Itraconazole inhibits fungal cytochrome P450-dependent 14α-demethylase, preventing ergosterol synthesis and fungal cell membrane formation. The combination was primarily studied to assess pharmacokinetic interactions; itraconazole is a strong CYP3A4 inhibitor and may impact the metabolism and exposure of AZD7986.
