Overview

Azelaic acid + fruit acid is a topical combination therapy used primarily in dermatology for the treatment of acne vulgaris, hyperpigmentation (such as melasma), and other skin conditions characterized by excess keratinization or pigment deposition. Azelaic acid is a naturally occurring dicarboxylic acid with antibacterial, anti-inflammatory, keratolytic (normalizing keratinization), and depigmenting properties. Fruit acids refer to alpha-hydroxy acids (AHAs) such as glycolic acid or lactic acid—compounds derived from fruits that act as chemical exfoliants to promote desquamation of the stratum corneum and improve skin texture. The combination leverages the complementary mechanisms of both agents: - Azelaic acid inhibits bacterial protein synthesis (notably against Cutibacterium acnes), reduces reactive oxygen species production for anti-inflammatory effects, normalizes follicular keratinization to prevent comedone formation, suppresses melanogenesis by inhibiting tyrosinase activity in abnormal melanocytes (reducing hyperpigmentation), and may reduce sebum production[2][3][4][5]. - Fruit acids/AHAs exfoliate dead skin cells from the epidermis, unclog pores, enhance penetration of other actives like azelaic acid, stimulate collagen synthesis for improved elasticity/texture, and help fade pigmentation[2][8]. Clinical studies have shown that combining azelaic acid with glycolic or lactic acids can be as effective as hydroquinone in treating facial hyperpigmentation in darker-skinned patients while also improving acne outcomes[10]. The combination may cause mild local irritation but is generally well tolerated.
