This combination therapy involves the macrolide antibiotic azithromycin and the beta-lactam antibiotic ampicillin, specifically investigated by Assiut University for the management of preterm premature rupture of membranes (PPROM). Azithromycin acts by binding to the 50S ribosomal subunit, inhibiting bacterial protein synthesis, while ampicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs). The regimen was studied to prolong the latency period (the time from membrane rupture to delivery) and reduce neonatal morbidity. A key clinical trial (NCT04202380) compared different azithromycin dosing protocols (a single 1000 mg dose versus a 5-day course) alongside standard ampicillin therapy. However, the published study from this trial was retracted due to significant concerns regarding data integrity, inconsistencies between the registry and the article, and a lack of evidence for prior ethical approval.
