Overview

This drug is a **combination of three antibiotics**: azithromycin (a macrolide), ampicillin (an aminopenicillin β-lactam), and sulbactam (a β-lactamase inhibitor). Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit. Ampicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins. Sulbactam is a β-lactamase inhibitor that extends the spectrum of ampicillin by protecting it from enzymatic degradation. The rationale for combining these agents lies in providing broader antibacterial coverage, synergistic bactericidal activity, and anti-inflammatory effects—especially in severe, multidrug-resistant infections. The combination aims to treat infections such as severe pneumonia or sepsis where an elevated inflammatory response plays a significant pathogenic role, with evidence in preclinical models for improved outcomes compared to β-lactam (ampicillin) monotherapy in some streptococcal infections[1][2][3][4].
