Azithromycin + loperamide is a combination therapy used primarily for the treatment of travelers' diarrhea. Clinical studies have demonstrated that this combination significantly reduces the duration of diarrhea compared to azithromycin alone. In a prospective, double-blinded, randomized trial conducted in Mexico, the combination therapy shortened the duration of diarrhea to approximately 11 hours compared to 34 hours with azithromycin monotherapy. The combination also resulted in fewer unformed stools within the first 24 hours post-treatment (1.2 versus 3.4 with azithromycin alone). Azithromycin is a macrolide antibiotic that targets bacterial pathogens causing diarrhea, while loperamide is an antidiarrheal agent that works by binding to mu-opioid receptors on intestinal muscles to decrease intestinal motility and reduce fluid loss. This combination is particularly effective in regions where Escherichia coli is the predominant cause of travelers' diarrhea.
