B-MAD chemotherapy is a combination chemotherapy regimen comprising brentuximab vedotin, methotrexate, L-asparaginase, and dexamethasone. This regimen is under investigation for the treatment of newly-diagnosed Extranodal NK/T-cell Lymphoma (ENKTL). Brentuximab vedotin is an antibody-drug conjugate (ADC) designed to selectively deliver a cytotoxic agent to CD30-expressing cancer cells. Methotrexate functions as an antimetabolite, inhibiting dihydrofolate reductase and thereby disrupting DNA synthesis. L-asparaginase is an enzyme that depletes asparagine, an amino acid crucial for the proliferation of certain cancer cells. Dexamethasone, a corticosteroid, exerts anti-inflammatory, immunosuppressive, and direct cytotoxic effects on lymphoid malignancies.
