B7-H3 CAR-T cell therapy is an investigational immunotherapy that utilizes genetically engineered T cells to target CD276 (B7-H3), a transmembrane protein member of the B7 family. B7-H3 is frequently overexpressed in a wide range of solid tumors, including pediatric neuroblastoma, glioblastoma, and various adult carcinomas, while maintaining limited expression in normal tissues, making it an attractive target for adoptive cell transfer. The therapy involves harvesting a patient's T cells, engineering them to express a chimeric antigen receptor (CAR) specific to B7-H3, and re-infusing them into the patient. Upon binding to B7-H3 on the tumor cell surface, the CAR-T cells undergo activation, proliferation, and exert cytotoxic effects through the release of perforins and granzymes. Several variations are in clinical development, including those with different costimulatory domains (e.g., 4-1BB or CD28) and those designed to overcome the immunosuppressive tumor microenvironment.
