Overview

B7-H4 CAR T cells are genetically engineered T cells expressing a chimeric antigen receptor (CAR) that targets B7-H4, an immune checkpoint protein highly expressed on multiple cancers, notably ovarian cancer. The CAR is designed to endow T cells with the ability to recognize and kill B7-H4-positive tumor cells independently of MHC presentation[1][2][3]. Preclinical models demonstrate that B7-H4 CAR T cells induce tumor regression in B7-H4-positive ovarian cancer xenografts; however, long-term engraftment can cause lethal off-tumor toxicity due to B7-H4 expression in normal tissues[1][2][3]. These findings suggest the modality has potent antitumor properties but also significant safety risks, notably delayed multi-organ toxicity. B7-H4 CAR T cell therapy is still experimental, with research primarily in preclinical and early translational stages.
