B7H3-targeted CAR-T cells are an investigational class of chimeric antigen receptor (CAR) T-cell therapies designed to target B7-H3 (CD276), a type I transmembrane protein and immune checkpoint molecule. B7-H3 is highly expressed across a broad spectrum of pediatric and adult malignancies—including neuroblastoma, osteosarcoma, glioblastoma, and triple-negative breast cancer—while maintaining limited expression in healthy tissues, making it an ideal therapeutic target. The therapy involves the genetic modification of a patient's T cells to express a CAR that typically utilizes a single-chain variable fragment (scFv) derived from monoclonal antibodies such as 8H9 or enoblituzumab. Once infused, these engineered T cells recognize and bind to B7-H3 on the surface of tumor cells, leading to T-cell activation, expansion, and the targeted lysis of the cancer cells. Clinical delivery methods vary by indication, including systemic intravenous infusion for solid tumors and regional delivery (e.g., intracerebroventricular or intrathecal) for central nervous system malignancies like diffuse intrinsic pontine glioma (DIPG).
