Bacille Calmette-Guérin (BCG) vaccine is a live attenuated strain of *Mycobacterium bovis* traditionally used for tuberculosis prevention. In a clinical program led by S. Andrea Hospital, it is being repurposed as an immunomodulatory therapy for radiologically isolated syndrome (RIS), a preclinical stage of multiple sclerosis. The vaccine is administered as a single intradermal injection and is hypothesized to prevent neuroinflammation and the progression of demyelinating lesions by inducing tumor necrosis factor (TNF) and modulating systemic immune responses. This approach aims to provide a safe, cost-effective, and accessible secondary prevention strategy for individuals at high risk of developing clinical multiple sclerosis.
