**Bacitracin zinc + lidocaine hydrochloride + neomycin sulfate + polymyxin B sulfate** is a fixed-dose topical OTC first-aid ointment combining three antibacterial agents with a local anesthetic. Bacitracin disrupts bacterial cell-wall synthesis by binding undecaprenyl pyrophosphate; neomycin binds the bacterial 30S ribosomal subunit and inhibits protein synthesis; polymyxin B disrupts susceptible Gram-negative bacterial membranes through interaction with lipopolysaccharide; and lidocaine reversibly blocks voltage-gated sodium channels to provide local analgesia. U.S. product labels describe use on the skin to help prevent infection and temporarily relieve pain or discomfort from minor cuts, scrapes, and burns. The exact four-ingredient topical combination should not be conflated with lidocaine 2% urethral jelly, which is a different formulation used for painful urethritis and urethral procedures.
