Bacterial autovaccine (autovaccuna) is a personalized, autologous immunotherapy being investigated for the management of chronic prosthetic joint infections (PJI), particularly in patients who are not candidates for curative surgical intervention. Developed by researchers at **IDIVAL** (Instituto de Investigación Marqués de Valdecilla) in Spain, the vaccine is prepared using inactivated bacterial strains isolated directly from the patient's own site of infection. It is administered sublingually to stimulate the mucosal-associated lymphoid tissue (MALT), aiming to induce a systemic and local immune response that can control bacterial load and inflammation. This approach is currently being evaluated in a Phase 3 clinical trial (AUTOPRO) as a potential alternative to long-term chronic suppressive antibiotic therapy (SAT).
