Overview

The Bacteroides cocktail is an investigational live biotherapeutic product developed by Baylor College of Medicine for the treatment of recurrent *Clostridioides difficile* infection (rCDI). Unlike conventional fecal microbiota transplantation (FMT), which utilizes screened donor stool, this product consists of a defined, standardized mixture of cultured *Bacteroides* species. The therapy is designed to restore the ecological balance of the intestinal microbiome and provide colonization resistance against *C. difficile* by outcompeting the pathogen for nutrients and niches. A Phase 1 clinical trial was conducted in Veterans Affairs (VA) patients to evaluate the safety and efficacy of the cocktail administered via oral capsules or colonoscopy; however, the study was terminated due to challenges in patient recruitment and manufacturing the defined bacterial mixture to required standards.
