BAFF-R CAR T cells represent a novel adoptive cell therapy where a patient's T cells are genetically engineered to express a chimeric antigen receptor (CAR) specifically designed to recognize and bind to the B-cell activating factor receptor (BAFF-R) expressed on malignant B cells. This immunotherapy aims to overcome limitations of existing CAR T cell therapies, particularly the issue of antigen loss (e.g., CD19-negative relapse) in B-cell malignancies. By targeting BAFF-R, which is a B-lineage marker essential for B-cell development and survival, these CAR T cells are designed to induce cytotoxicity against various B-cell lymphoid malignancies, including lymphomas and leukemias, and are also being explored for autoimmune diseases. The therapy involves isolating T cells, modifying them to express the BAFF-R CAR, expanding them ex vivo, and then reinfusing them into the patient. Different constructs exist, some incorporating co-stimulatory domains like 4-1BB or CD28, and some experimental versions may target additional receptors like BCMA and TACI to enhance efficacy and prevent antigen escape.
