Overview

Bamifylline is a methylxanthine derivative and selective A1 adenosine receptor antagonist primarily used as a bronchodilator in the treatment of respiratory conditions such as bronchial asthma and chronic obstructive pulmonary disease (COPD)[1][2][4][6]. By blocking A1 adenosine receptors on bronchial smooth muscle cells, it relaxes airway smooth muscle and reduces bronchospasm[2][7][8]. Unlike theophylline (another xanthine), bamifylline generally lacks significant central nervous system stimulant effects[6]. It is rapidly absorbed after oral administration and has an elimination half-life of approximately 2 hours (though some sources report up to 17.5 hours), with primary excretion via urine[6][8]. The drug is typically administered twice daily due to its pharmacokinetic profile. Its main indications are for relief of symptoms associated with asthma and COPD such as wheezing and shortness of breath[1].
