Banisteriopsis caapi extract is a botanical preparation derived from the South American vine traditionally used to prepare the psychoactive brew ayahuasca. The extract is characterized by a high concentration of beta-carboline alkaloids, primarily harmine and harmaline. Pharmacologically, these compounds act as potent, reversible inhibitors of monoamine oxidase A (MAO-A) and have also been identified as NMDA receptor antagonists. In clinical research, the extract has been investigated for its potential to alleviate motor symptoms in drug-naive (de novo) patients with Parkinson's disease. Results from randomized, placebo-controlled trials indicate significant improvements in motor function as measured by the Unified Parkinson's Disease Rating Scale (UPDRS). The therapeutic effect is hypothesized to stem from the modulation of striatal pathways and the correction of neurotransmitter imbalances. Common adverse effects include nausea, diarrhea, and transient exacerbation of resting tremors.
