Overview

The Basel phenotyping cocktail is a specialized combination of low-dose probe drugs designed for simultaneous assessment of multiple cytochrome P450 (CYP) enzyme activities, which are crucial for drug metabolism. It contains caffeine (10 mg) for CYP1A2, efavirenz (50 mg) for CYP2B6, flurbiprofen (12.5 mg) for CYP2C9, omeprazole (10 mg) for CYP2C19, metoprolol tartrate (10 mg) for CYP2D6, and midazolam (2 mg) for CYP3A4. The cocktail was developed to improve clinical applicability of CYP phenotyping by using probe drugs widely available in clinical practice. A significant innovation is its formulation as a single capsule ("CombiCap"), simplifying administration and allowing smaller drug amounts compared to previous methods. It has been extensively validated through clinical studies, demonstrating tolerance, lack of mutual interactions, and the potential for simplified sampling protocols and alternative sampling methods like dried blood spots and saliva for certain CYP isoforms.
