Bavituximab + sorafenib is a combination therapy investigated primarily for advanced hepatocellular carcinoma (HCC). Bavituximab is a chimeric monoclonal antibody that targets phosphatidylserine, a membrane phospholipid externalized on tumor and endothelial cells, especially in response to stressors such as chemotherapy or targeted agents like sorafenib. By binding to phosphatidylserine, bavituximab modulates the tumor microenvironment and enhances anti-tumor immune responses. Sorafenib is an oral small molecule multikinase inhibitor that targets several tyrosine protein kinases involved in tumor cell proliferation and angiogenesis, including RAF kinases and vascular endothelial growth factor receptors (VEGFR). The rationale for combining these agents stems from preclinical data showing that sorafenib increases the exposure of phosphatidylserine on cancer cells, potentially enhancing the immunomodulatory effects of bavituximab. Clinical studies have shown that this combination can be administered safely; however, efficacy results have been inconclusive with no significant improvement in overall survival or response rates compared to historical controls using single-agent sorafenib[1][3]. The most common adverse events reported were diarrhea, fatigue, and anorexia[1].
