BAY86-5277 is a small molecule antagonist of the C-C Chemokine Receptor type 1 (CCR1), originally developed by Bayer AG. It works by blocking the interaction between CCR1 and its chemokine ligands, such as CCL3 (MIP-1α), which inhibits the chemotaxis and infiltration of monocytes, macrophages, and T cells into inflamed tissues. This mechanism is intended to mitigate the hyperinflammatory response, or "cytokine storm," associated with severe respiratory infections. BAY86-5277 has been evaluated in a Phase II clinical trial for patients hospitalized with COVID-19 to determine if reducing pulmonary inflammation can improve clinical outcomes. Historically, CCR1 antagonists as a class were investigated for autoimmune conditions like rheumatoid arthritis and multiple sclerosis, though BAY86-5277's primary recent focus has been on acute inflammatory states.
