BB4 antibody + iodine 131 is a radioimmunoconjugate composed of the monoclonal antibody BB4 (which targets syndecan-1/CD138) conjugated to the radioactive isotope iodine-131. The combination is designed for radioimmunotherapy of cancers expressing high levels of CD138/syndecan-1, such as multiple myeloma and certain breast cancers. The mechanism involves the specific binding of the BB4 monoclonal antibody to CD138 on tumor cells; once bound, the attached radioactive iodine delivers localized radiation to kill cancer cells while sparing most normal tissues. Preclinical and early clinical studies have shown promising antitumor activity and manageable toxicity profiles in models of multiple myeloma and breast cancer[1][2][3].
