Overview

BCG Japan, specifically the Tokyo 172 substrain, is a live-attenuated preparation of *Mycobacterium bovis*. It is classified as an "evolutionarily early" substrain, meaning it retains certain genetic regions (such as RD2) that were lost in later-derived strains like Tice or Connaught. Developed and manufactured by the Japan BCG Laboratory, it is used globally both as a vaccine for the prevention of tuberculosis (typically administered percutaneously) and as an intravesical immunotherapy for non-muscle invasive bladder cancer (NMIBC). In the context of bladder cancer, BCG Japan induces a robust local inflammatory response characterized by the recruitment of T cells, natural killer cells, and macrophages, and the production of various cytokines (including IL-6, IL-8, and TNF-alpha), which collectively mediate the destruction of urothelial tumor cells.
