Overview

BCG Phipps is a specific substrain of the Bacillus Calmette-Guérin (BCG) vaccine, derived from *Mycobacterium bovis*. It is classified as an evolutionarily 'late' substrain, characterized by the genetic deletion of the Region of Differentiation 2 (RD2) and the presence of surface lipids such as phthiocerol dimycocerosates (PDIM) and phenolic glycolipids (PGL), which are associated with its virulence and immunogenic profile. Clinically, BCG Phipps is utilized as a biological response modifier for the intravesical treatment of non-muscle invasive bladder cancer (NMIBC), particularly carcinoma in situ. Its mechanism of action involves the induction of a localized, robust immune response within the bladder wall, leading to the production of pro-inflammatory cytokines like interleukin-6 (IL-6) and interleukin-8 (IL-8) and the recruitment of immune effector cells to eliminate tumor cells. While used in oncology and tuberculosis prevention, research suggests its immunostimulatory potency may vary compared to other BCG substrains like Connaught or Russian.
