Overview

BCG042 is a fully human IgG1 monoclonal antibody targeting Leukocyte Immunoglobulin Like Receptor B4 (LILRB4), developed by Biocytogen for the treatment of monocytic acute myeloid leukemia (AML). LILRB4 is an immunosuppressive receptor highly expressed on monocytic AML blasts that facilitates immune evasion and tissue infiltration through interaction with APOE and subsequent SHP-1/2 signaling. BCG042 is engineered with an Fc mutation to enhance antibody-dependent cellular cytotoxicity (ADCC), promoting the targeted clearance of AML cells. Preclinical studies demonstrate that BCG042 binds to LILRB4 with nanomolar affinity, inhibits APOE-LILRB4 signaling, and induces significant tumor regression in xenograft models, outperforming analogs of other LILRB4-targeted agents like IO-202.
