This is a combination therapy consisting of a BCL-2 inhibitor (most commonly venetoclax) administered together with the HyperCVAD chemotherapy regimen. The HyperCVAD regimen is an intensive chemotherapeutic protocol that alternates cycles of hyperfractionated cyclophosphamide, vincristine, doxorubicin, and dexamethasone (Cycle A) with high-dose methotrexate and cytarabine (Cycle B). The addition of a BCL-2 inhibitor targets the anti-apoptotic protein B-cell lymphoma 2 (BCL-2), promoting apoptosis in malignant lymphoid cells. This combination has been investigated primarily for acute lymphoblastic leukemia (ALL), including both T-cell and B-cell subtypes, especially in relapsed or refractory settings. Preclinical and early clinical studies suggest that combining venetoclax with low-intensity or standard-intensity HyperCVAD regimens may enhance antileukemic activity due to synergistic effects on leukemic cell survival pathways[3][9][1].
