BCMA bispecific antibodies represent a class of T-cell engaging immunotherapies primarily developed for the treatment of multiple myeloma. These molecules are engineered with dual binding specificity: one domain targets the B-cell maturation antigen (BCMA), which is highly and selectively expressed on the surface of malignant plasma cells, while the other domain binds to the CD3 receptor on T cells. By simultaneously engaging both targets, the antibody facilitates the formation of an immunological synapse between the cytotoxic T cell and the myeloma cell. This interaction triggers T-cell activation, degranulation, and the release of perforins and granzymes, leading to the targeted lysis of the BCMA-expressing tumor cells. Several agents within this platform, such as teclistamab and elranatamab, have received regulatory approval for patients with relapsed or refractory multiple myeloma who have received multiple prior lines of therapy.
