Overview

BCMA CAR-CXCR4R334X NK cells are a genetically engineered natural killer (NK) cell therapy designed to treat multiple myeloma. These NK cells are modified to express a chimeric antigen receptor (CAR) that targets B-cell maturation antigen (BCMA), which is highly expressed on malignant plasma cells in multiple myeloma. Additionally, they are engineered to express a mutant CXCR4 receptor (CXCR4^R334X), which enhances the cells' homing and migration toward the bone marrow microenvironment by increasing responsiveness to the chemokine CXCL12/SDF-1α. This modification boosts bone marrow infiltration, in situ activation, and the cytolytic function of NK cells against BCMA-positive tumor cells. Preclinical studies indicate increased anti-myeloma efficacy compared to unmodified or standard BCMA-CAR NK cells. The key mechanistic advantage is enhanced tumor site localization, cytotoxic response, and improved CAR signaling independent of external CXCL12 stimulation.
