BCMA-CD19 nanobody-based compound CAR-T (ncCAR) is an investigational dual-targeting chimeric antigen receptor (CAR) T-cell therapy developed by iCell Gene Therapeutics. Unlike conventional CAR-T therapies that utilize murine-derived single-chain fragment variables (scFv), this platform employs camelid-derived nanobodies (VHH) to target both B-cell maturation antigen (BCMA) and CD19. This dual-antigen approach is designed to eliminate both CD19+ B cells and BCMA+ long-lived plasma cells, which are considered the root cause of autoantibody production in many autoimmune diseases. The therapy is manufactured using the proprietary Neo-G process, which is designed to shorten manufacturing time by 3-4 days and enhance efficacy. It is currently being evaluated in Phase I clinical trials for systemic lupus erythematosus (SLE) and has demonstrated clinical activity in multiple myeloma and immune thrombocytopenia (ITP).
