Overview

BCMA-CD38 bispecific CAR-T cells (such as the clinical candidate BM38) represent a dual-targeting chimeric antigen receptor (CAR) T-cell therapy designed to treat relapsed or refractory multiple myeloma (R/R MM). By simultaneously targeting B-cell maturation antigen (BCMA) and CD38, two antigens highly expressed on malignant plasma cells, this bispecific CAR-T therapy aims to overcome antigen escape and tumor relapse associated with single-target CAR-T therapies. The therapy utilizes autologous T cells genetically engineered to express a tandem bispecific CAR construct, typically incorporating a 4-1BB co-stimulatory domain and a CD3ζ signaling domain. Clinical development, including Phase 1 trials (such as ChiCTR1900026286 and ChiCTR1800018143) conducted by institutions like Jingzhou Central Hospital of Yangtze University and Union Hospital of Huazhong University of Science and Technology in collaboration with Cellyan Therapeutics, has demonstrated encouraging efficacy with high rates of complete response and manageable safety profiles in patients with R/R MM.
