Overview

BCMA-directed CAR-T (B-cell maturation antigen-directed chimeric antigen receptor T-cell) therapy is a specialized form of adoptive cell immunotherapy primarily used for treating relapsed or refractory multiple myeloma. The process involves collecting a patient's T cells and engineering them to express a CAR that specifically binds to BCMA, a protein highly and nearly universally expressed on malignant plasma cells. Once re-infused, these cells undergo activation and expansion upon encountering BCMA, resulting in the direct lysis of myeloma cells. This platform includes commercially approved therapies like idecabtagene vicleucel (Abecma) and ciltacabtagene autoleucel (Carvykti), as well as various investigational constructs featuring modifications such as cytokine secretion (e.g., IL-18) or CRISPR-mediated silencing of inflammatory cytokines (e.g., IL-6, IL-1B, GM-CSF) to enhance efficacy and reduce the risk of cytokine release syndrome.
