Overview

**BCMA-directed CAR T-cells + CD19-directed CAR T-cells** is a combination cell therapy involving the sequential or simultaneous administration of chimeric antigen receptor (CAR) T-cells engineered to target B-cell maturation antigen (BCMA) on myeloma cells and CD19 on B-lineage cells, primarily developed to treat relapsed/refractory multiple myeloma (RRMM), particularly cases with extramedullary infiltration or risk of antigen escape. BCMA CAR T-cells recognize BCMA (TNFRSF17), a TNF receptor superfamily member expressed on plasma cells, triggering T-cell activation via CD3ζ and co-stimulatory domains (e.g., 4-1BB or CD28) for cytotoxic killing of myeloma cells, while CD19 CAR T-cells target the B-lymphocyte antigen CD19 to eliminate residual malignant B-cells and prevent relapse due to BCMA downregulation. This dual-targeting approach addresses limitations of single-target BCMA therapies like idecabtagene vicleucel (Abecma) or ciltacabtagene autoleucel (Carvykti) by mitigating antigen loss, with clinical trials demonstrating safety profiles focused on dose-limiting toxicities and cytokine release syndrome.[2][3]
