A BCMA x CD3 bispecific antibody is a therapeutic agent designed to bridge T cells and plasma cells by simultaneously targeting the B-cell maturation antigen (BCMA) and the CD3 subunit of the T-cell receptor complex. This dual binding triggers T-cell activation and directed cytotoxicity against BCMA-expressing cells, which include malignant plasma cells in multiple myeloma and pathogenic plasma cells in autoimmune or alloimmune conditions. While several agents in this class, such as teclistamab and elranatamab, are FDA-approved for relapsed or refractory multiple myeloma, they are also being investigated for non-oncology uses. Specifically, clinical research led by institutions like West China Hospital is evaluating their efficacy in desensitizing highly sensitized kidney transplant candidates and treating chronic active antibody-mediated rejection by depleting the plasma cells responsible for producing donor-specific antibodies (DSAs).
