A BCMAxCD3 bispecific T-cell engager (TCE) is a therapeutic modality designed to treat hematologic malignancies by redirecting the immune system's T-cells to attack cancer cells. This bispecific molecule simultaneously binds to the B-cell maturation antigen (BCMA), which is highly expressed on the surface of multiple myeloma cells and certain acute myeloid leukemia (AML) cells, and the CD3 receptor on T-cells. This dual engagement facilitates the formation of a cytolytic synapse, leading to T-cell activation and the subsequent release of perforin and granzymes, which induce apoptosis in the target tumor cells. While several BCMAxCD3 TCEs, such as teclistamab and elranatamab, are approved for the treatment of relapsed or refractory multiple myeloma, emerging research highlights BCMA as a viable target in AML, suggesting potential for clinical expansion into myeloid diseases.
