Overview

BCMAxGPRC5D CAR T (BMS-986458) is an investigational dual-targeted chimeric antigen receptor (CAR) T cell therapy developed by Bristol Myers Squibb for the treatment of relapsed/refractory multiple myeloma. The therapy is engineered to simultaneously target two distinct antigens expressed on malignant plasma cells: B-cell maturation antigen (BCMA) and G protein-coupled receptor class C group 5 member D (GPRC5D). By utilizing a tandem or bicistronic CAR construct, the therapy aims to enhance the depth and durability of clinical responses while mitigating the risk of disease relapse driven by antigen escape, a common resistance mechanism in single-target CAR T therapies. GPRC5D is highly and specifically expressed on myeloma cells independently of BCMA, providing a complementary target for patients who may have lost BCMA expression. The therapy is currently undergoing evaluation in Phase 1 clinical trials.
