The Beach regimen is an experimental multi-drug combination therapy under investigation for the treatment of relapsed or refractory acute myeloid leukemia (R/R AML). This hexal-combination protocol consists of a BCL-2 inhibitor (targeting the anti-apoptotic BCL-2 protein), Zemestosita (likely Zemestostat, an EZH2 inhibitor), azacitidine (a hypomethylating agent), cytarabine (a nucleoside analog chemotherapy), chidamide (an HDAC inhibitor), and G-CSF (granulocyte colony-stimulating factor). The regimen is designed as a salvage induction therapy, combining targeted apoptosis induction, epigenetic modulation, and cytotoxic chemotherapy to overcome resistance in difficult-to-treat leukemic populations. G-CSF is included to support granulocyte recovery and potentially sensitize leukemic blasts to the cytotoxic effects of the chemotherapy. It is currently being evaluated in a Phase II clinical trial (NCT07651475) led by Liping Dou at the Chinese PLA General Hospital.
