BEAM-ATG is a high-dose chemotherapy conditioning regimen followed by autologous hematopoietic stem cell transplantation (ASCT). It is composed of the BEAM regimen—carmustine (BCNU), etoposide, cytarabine (Ara-C), and melphalan—supplemented with anti-thymocyte globulin (ATG). This combination is designed to achieve profound immunoablation by utilizing the cytotoxic effects of alkylating agents (carmustine, melphalan), a topoisomerase II inhibitor (etoposide), and an antimetabolite (cytarabine), alongside the T-cell depleting properties of ATG. While BEAM is a standard conditioning protocol for lymphomas, the BEAM-ATG variant is specifically utilized in the treatment of highly active or therapy-refractory autoimmune diseases, most notably multiple sclerosis, to reset the immune system.
